OnPatient Instructions


  • After payment, you will receive an email from our patient portal, OnPatient, directing you to their site. You will need to create an account. This is how we conduct our telemedicine visits and collect HIPAA protected private health information.

  • Next, upload a drivers license or photo, answer our questionnaire, and sign the consent and HIPAA forms.

  • This process takes some time, you should be prepared to spend 15-30 minutes reading and completing the forms.

  • The completion of the OnPatient forms is a crucial step in the process and is required prior to your telemedicine appointment to determine eligibility.

Click Check In. You can also book new appointments from this page.

Please upload a recent photo, an ID card such as drivers license is preferred.

Provide a list of all active medical conditions, medications, and allergies.
