How to Make Ketamine Therapy Results Last: An Integration Guide

Ketamine opens a neuroplasticity window — but what you do with it determines how lasting the results are. Here's your guide to integration for long-term healing.

Dr. Ben Soffer

Ketamine therapy can produce rapid, remarkable shifts in mood, perspective, and emotional pain. But if you've heard about people whose relief faded after a few weeks, you've identified one of the most important factors in long-term outcomes: integration.

Integration is the work you do between and after sessions to translate the insights, openings, and neurological changes from ketamine into lasting transformation. Think of ketamine as creating a window — a period of heightened neuroplasticity where your brain is unusually receptive to change. Integration is what you build during that window.

Understanding the Neuroplasticity Window

After a ketamine session, your brain experiences a surge in BDNF (brain-derived neurotrophic factor) — often called "Miracle-Gro for the brain." This promotes the growth of new synaptic connections, particularly in areas of the prefrontal cortex that regulate mood, self-reflection, and emotional regulation. This window of heightened neuroplasticity typically lasts 24–72 hours after each session, with some research suggesting effects on neural remodeling persist for 1–2 weeks.

During this window, the brain is literally more capable of forming new patterns. Positive experiences, new thoughts, therapeutic insights, and healthy behaviors undertaken in this period have an outsized chance of becoming "sticky" — of being encoded as new neural pathways that persist over time.

Journaling: Capturing What Surfaces

Many patients experience profound insights, vivid imagery, emotional releases, or new perspectives during ketamine sessions. These can fade quickly if not captured. Keep a journal specifically for your ketamine experiences, and write in it as soon after a session as you're able — even brief notes on your phone immediately after are valuable.

Useful journaling prompts include:

  • What images, memories, or feelings came up during the session?
  • Was there a moment of clarity or insight — something that felt important?
  • What do I want to let go of? What do I want to bring more of into my life?
  • What feels different today compared to before I started treatment?
  • What one small action could I take this week that aligns with what I learned?

Therapy: The Most Powerful Amplifier

Research consistently shows that ketamine paired with psychotherapy produces better long-term outcomes than ketamine alone. You don't necessarily need a therapist who specializes in ketamine-assisted therapy (though that's ideal) — any good therapist you trust can help you process what arises in sessions.

Schedule a therapy session within 24–72 hours of a ketamine session if possible. This is when the neuroplasticity window is most open and the material from your experience is freshest. Bring your journal notes. Let the session flow from what emerged rather than sticking to your usual topics.

If you don't currently have a therapist, this is worth investing in. Even monthly sessions during a ketamine course can meaningfully extend results.

Mindfulness and Meditation

Mindfulness practices are natural companions to ketamine therapy. Both create a quality of nonjudgmental awareness — observing thoughts and feelings without being consumed by them. Regular meditation during a ketamine course has been shown to improve outcomes in several studies on psychedelic-adjacent therapies.

You don't need an elaborate practice. Even 10–15 minutes of guided meditation or breathwork daily during your treatment period can support the neuroplasticity benefits. Apps like Insight Timer, Calm, or Headspace offer free and accessible starting points.

The period immediately after a session, when you're still in a softened, open state, can be an especially powerful time for a brief, gentle meditation — even just resting with your eyes closed and breathing intentionally.

Lifestyle Factors That Support Neuroplasticity

The same behaviors that support brain health in general also amplify ketamine's neuroplastic effects:

  • Sleep: BDNF consolidation and synaptic pruning happen predominantly during sleep. Protecting your sleep during a ketamine course isn't optional — it's therapeutic. Aim for 7–9 hours.
  • Exercise: Aerobic exercise is one of the most potent natural stimulants of BDNF. A 30-minute walk or jog the day after a session actively supports the neuroplastic changes ketamine initiated.
  • Nutrition: Anti-inflammatory foods (Mediterranean-style diet, omega-3s, reduced sugar) support brain health. Alcohol should be avoided during active treatment periods.
  • Social connection: Isolation reinforces depression. Use the post-session window to reach out to people you care about, even briefly. Connection is deeply reparative.

Reducing Old Patterns: The "What to Avoid" List

Integration isn't only about adding positive practices — it's also about being intentional about not reinforcing old patterns during the neuroplasticity window:

  • Avoid excessive alcohol, which suppresses BDNF and neural remodeling
  • Limit doomscrolling and passive social media consumption, which can reinforce anxious thought patterns
  • Avoid highly stressful, confrontational situations in the 48 hours after a session if possible
  • Notice when your inner critic flares up and practice redirecting gently

Setting an Integration Intention Before Each Session

Before each session, take a few minutes to set an intention — not a demand or expectation, but a gentle direction. "I'd like to understand why I feel so disconnected from my family." "I'm open to whatever needs to be seen." "I'd like to feel more like myself again."

Intentions help orient the experience and give you something to reflect on afterward. They don't control what happens — ketamine is not that predictable — but they signal to your mind what you're showing up for.

How Long to Maintain Integration Practices

The short answer: as long as they're serving you. For most patients, we suggest maintaining active integration practices — journaling, therapy, regular exercise, mindfulness — for at least 4–8 weeks after completing a ketamine course. For many, these practices become permanent parts of their lives, and that's the goal.

Ketamine opens a door. Integration is how you walk through it — and build something worth keeping on the other side.
