Savella (milnacipran) has a pronounced discontinuation pattern driven by its very short half-life. In the US it is FDA-approved for fibromyalgia rather than depression. Patients tapering Savella often experience a return of both fibromyalgia symptoms and the discontinuation syndrome itself, which can be hard to separate.

Tapering guidance

  • A typical taper reduces by 25 mg per dose every 1-2 weeks. Savella is usually dosed twice daily, so a typical schedule is reducing one of the two daily doses first, then the other.
  • Coordinate any taper with the prescribing physician, particularly for fibromyalgia patients whose pain may return.

Where ketamine therapy fits

Savella is compatible with at-home ketamine therapy. Ketamine has evidence in both depression and chronic pain (including fibromyalgia and centralized pain syndromes), so the conversation about ketamine in a Savella context often spans both indications. Continue Savella as prescribed unless the prescribing physician advises otherwise.
